Although conventional procedures show fine results, continuous technical optimization should be the goal. Open surgeries for cervical spine has a high risk of many other complications such as posterior laminoforaminotomy which violates posterior neck muscles that causes long-lasting neck discomfort post operatively. With the advent of minimally invasive cervical spine surgery techniques, surgeons can avoid the risk of varied complications of ACDF under local anesthesia with less than 4mm single portal high-resolution working channel using 2.1 mm MaxMoreSpine MINI endoscope through 5 mm skin incision and various forceps & special instruments which have been regarded as an effective treatment modality in selected cases. And special designed threaded circular cannular and obturator allow a meticulous insertion, thus avoiding the use of hammer while inserting them which puts the patients on the risk due to sudden uncontrolled advancement of them into the disc space and can penetrate spinal cord leading to devastating consequences. The high-resolution working channel MaxMoreSpine endoscope allows surgeons to selectively remove the cervical herniated disc. Even though the minimal postoperative disc height reduction was observed but significant effect on therapeutic success as well as the sagittal alignment and segmental motion was well preserved.
